I hereby contract for water service from the Yarmouth Water District for service at the above referenced location. I agree to comply with all applicable Rules and Regulations of the Maine Public Utilities Commission and the Yarmouth Water District. I further agree to pay for all Water District charges as stated in the Terms and Conditions of the Yarmouth Water District, copies of which are on file at the District’s office. I understand that failure to make payment may result in disconnection of water service which will cause any life safety fire sprinkler system to be inoperable if installed. An account set-up fee of $10.00 will be applied to your first bill.
